1. se vuoi fare la modella e non sai l'inglese non è buona cosa
2. se ti firmi con il cognome prima del nome, nella maggior parte dei casi la domanda finisce direttamente nel cestino
3. può essere circa così:
dear sirs,
my name is xx xx and i'm writing you in order to have some information about the application i sent you some (days/weeks/months) ago by internet form. I would like to know if my application has been accepted.
I look forward to your reply.

xx xx
La traduzione non è letterale e frase per frase in quanto la tua versione risulta un po' ridondante per l'uso inglese nella corrispondenza.
