Not a translation word by word but as good as I good do ;-):

I'm not able to translate this, but as far as I think, it's lymphadenopathy of the cervical (neck?) region - the lymph nodes at the cervix/neck are swollen

and a second diagnosis intestinale dysbiose: Dybiose is a fungus (don't know if this is the correct translation) in the gut

I guess you are in the army stadioned in Germany ? If so , than Tricare has to take care of that . It is probably just a bill , you shoudn't have to pay it , unless you had no referral . They want you to transfer what you owe , within 14 days , for whatever amount they put down . You can also do that at one of the banks on post . That has nothing to do with your illness - it's just billing information. It means: please include (maybe a order number?) with your payment - payment must be received within 14 days - for my work I will bill you ....(amount)
