Yes it is sometimes it's considered a genetic mutation or a disruption with genes...
Sometimes it's cause of nuclear radiation or radiation on the fetus and the mother.

Yes, it usually occurs with a mutation in a HOX gene, which determines pretty much where and what as far as genotipic expression. However they would probably not be of good use.
