I have a better idea - why don't you have a go at the letter yourself first, and then ask people on here if it's formal enough? It's very difficult to just give you a list of formal words without knowing exactly what you want to write, plus it'll sound better if whatever it is you want to say comes from the heart, not a website. :-)

Write every word out... for example:
Use "do not" instead of "don't"

Also, use transition words such as "furthermore, moreover, in addition".

Go to www.dictionary.com and use the thesaurus to find some cool words with the same meaning as more common words. Make sure you look up the definitions or you'll sound like a complete moron if you use a word incorrectly.

Write the letter in your own words and I will try to improve it if I can.

I would like to thank you for the gift that you gave me. It was very appreciated and very kind to think of me in such a special way.

I don't think that you have to write a more formal note than that. This will open up the door between you and the teacher for any future dialogue. If you wish to express your appreciation for her class or the way she teaches do this in person.

It's difficult to suggest any words if we don't know the subject of the letter.

Would be glad to. But need to know what letter is about before I can give you that information.
