menthol is a chemical, it's properties make it a pain reducer, most cough drops and sprays use menthol to deaden nerve endings temporarily.
Menthol cigarettes are the same way, they are "smoother" because the menthol deadens the mouth and throat so the smoke isn't harsh on the senses.
In icy hot it has similar effects, it is a numbing agent, other chemicals are used to create the sensation of heat and ice as well, it actually does make the area warmer
