I had to go to the county clerk of the state where I was born. Cost 15 for mine. They say you can do it by mail if you're out of state but I never did that. I drove.

You have to get it from the county seat of the county where you were born, or the state capital. You can order it by phone or on the Internet, and you'll have to send them money for it.
