There are a couple of criteria used. The first is an absolute email address. If email comes in from any one of the few million addy's that are known spam, it goes to Bulk.

Secondly, if your name is in a list of numerous addy's on the email, the email is listed as suspicious, and goes through the rest of the processing.

The processing that takes place analyzes the email contents. If there are words like prescription drug names, porn terms, or other high spam words, the email is marked as spam, and put in to Bulk.

You should be able to mark an email as not spam and have them come to your Inbox from then on.

If you look at the headers most of them show as bulk mail and that is one of the filters they use. I am almost certain they are using a SURBL and MAPS server to help identify spammers also. That used in conjunction with what you say is spam is what they are using more than likely.
