No one can ever be thanked enough, so redundancy is not an issue. A smile and nod is a nice gesture, but a second verbal "thank you" is also completely appropriate. Not doing anything is also acceptable as long as the person's effort has been acknowledged by one of you.

Courtesy and polite behavior can rarely be overdone. In addition to or in spite of whatever etiquette might be appropriate, since you were the adult, and the staff member was saving your time as well, you should have in some way conveyed your appreciation, whether it's "thanks again" or "much obliged" or "I appreciate your help" or even just strong eye contact, a nod and a smile. But yes, you as the adult should have responded.

Civility is always admired. Since you are together and the attendant have no way of knowing who actual require the assistant.
I am sure any gratitude display to store attendant is appreciated, so yes say thank you as well and smile as you are saying it.

When my friend and I shop together we both say thank-you. It's not like it requires a lot of effort.

A Thanks wouldn't have been out of order -- If someone I am with asks a question and then thanks the responder, I generally either say Thanks or Smile and Nod (Acknowledging the person) or say something like "Have a Nice Day!"

I would if the staff memeber looks directly at you and you make eye contact - otherwise there's no need.
