When filling out an application form, what is the best way to deal with changes in the company name/address of a former employer?

If a potential employer can't verify past workplaces, will he/she think I've lied about my work history?

You could put their current name/address, and under that, state "formerly known as (previous name) or formerly located at (previous location)."

On a separate sheet of paper type the name and addresses of former employers.

Unless the former employer is no longer in business, they should be able to verify your work history.
