The taste and quality of food is foremost. If I do not enjoy my dish, I will probably not come back to try another. Soup, salad, and bread quality is also important. All the little things add up too.
Secondly, a comfortable atmosphere; comfortable seating where everyone is not on top of one another. Thirdly, a good value for my money; if the entrees are $20+, they better taste like it. Lastly, good customer service; I do not want to sit for 10 minutes before the wait person asks for our drink order, and then is unfriendly to boot!

obviously all of the above, food and service are the most important things, but...sometimes me & my friends / family will revisit a resturant because it is unique in some way. Maybe it will have a breathtaking view, a singing chef, cooking demonstrations (open kitchnes are always a winner) etc etc, if there is some kind of entertainment it can draw a crowd even though the place next door maybe serving better food. Depending on the pricing you expect different things, i wouldn't want a signing chef at a very up market place but maybe an underfloor aquarium would make me take my friends there? Add to this a great wine list and there are all ingredients for success.

Great food and service for sure. I also look for a place that has something unique to offer - not the "same old, same old". Service is a big one for me though. The food can be great, but if I'm being served by someone w/an attitude, or they can't be bothered to check on me during the meal, I won't be back.

It depends on how good the food was, how fast the service was, how pleasant the staff was, how clean the bathrooms were and the overall environment

GREAT food, good service, nice atmosphere and floor space, cleansiness, prices, friendly staff (host/hostesses/servers), availability, overall attractive ambiance.
