I am a professional chef; I can tell you that if the onion is going to be cooked, generally speaking you use the white bulb and the white part of the stalk, chopping until the color changes to nearly all green and starts to become hollow. This is because the bulb and white stalk are solid onion, and hold up in a sauce or in a saute. If it is going to be for a garnish, then cutting the green hollow stalk into little rings is fine. They just don't hold their shape well when cooked and become slimy.

I usually just use the bulb unless the dish could use some color then I use the whole thing. I don't think that it effects the flavor at all one way or the other~of course that is just my opinion. You or others may disagree. I also use the green stems/whatever if the bulb is pretty small. Um-mm green onions, I like to just eat them along with the meal, sandwich or whatever. Sounds good, wish I had some!!
Use all of it, it will be good!!

I use the green only to just a little above the top of the white, then I take the white root part and put it in a small glass of water so the green part will grow again.

usually the recipe will specify what part of the green onion to use. If I find a recipe like you have though, I will just chop and use the whole thing. I'm pretty sure whatever your making will be fine no matter what part of the onion (or whole thing) you decide to use.

you can us the whole onion, the green blades and the white bulb at the end. Just be sure to remove the outside on the while bulb, and chop finely.

Depends on what you are cooking. Things that require a hot or long cook time the greens are usually not added unitil the end (think chives). In quick cooking (stir frys) the entire onion can and is used.

You use the green and the white. Just slice the white very thinly. Both have a mild onion flavor.
