There are many ways a student can fail, the most common take place before the examiner asks the first question:
The logbook does not have the necessary PIC cross country hours in it, or the forms are not filled out correctly.

After that, the examinaer wants to make sure you know what you need to know, and not just by rote, but are able to apply the knowledge to situations.

Not knowing what is real since so much is fudged during rides. I departed without a clearance because I always filed when I practiced and I didn't know I was supposed to ask my examiner for a clearance. He didn't fail me on it though. Also make sure you know your plates and consider weather implications (like a strong wind making the approach with the lowest minimums unuseable).

I used to have a book called "Eye of the Examiner". It was written by Howard Fried. It was a compilation of articles he wrote about oral and practical exams. Maybe you can find it on the internet.
