Visas are issued by the consulate of the country you are travelling to, if you are going to be in excess of 90 days their.
You can apply for a passport at nearly any post office, but you must give at least three weeks time for an expedite processing and up to 8 weeks for a standard.

You need a passport to go to any country, esp. after 9/11.
You have to fill out an application and bring two wallet-size photos of your face to the U.S. Immigration office.
